Policies & Procedures

1. Purpose of Chiropractic Care
 

The purpose of chiropractic care in our office is not to medically diagnose or treat diseases, suppress symptoms, perform surgery, or prescribe medications. Our goal is to:

​

  • Improve the health and function of your spine and nervous system​

  • Support your body’s natural ability to function and heal at its highest potential


It is our sole objective to help improve and maintain the health and function of your spine and nervous system to the maximum degree possible for you.
 

2. Purpose of Functional Medicine

 

Functional medicine evaluations at our clinic are to assess the body’s potential nutritional needs. Please note:

​

  • These evaluations do not diagnose or treat disease or specific medical conditions.

  • The nutritional or dietary recommendations are not guaranteed, and no assurances have been made.

  • These services are not a substitute for professional medical advice or treatment from your medical doctor.
     

Important Legal Disclosure:

 

According to the Federal Food, Drug, and Cosmetic Act, Section 201(g)(1), the term “drug” is defined as:

“Articles intended for use in the Diagnosis, Cure, Mitigation, Treatment or Prevention of disease.”

​

A vitamin, mineral, trace element, amino acid, herb, or homeopathic remedy is not classified as a drug, even if it may affect a disease process or symptom. Therefore:

​

  • Any suggested nutritional or dietary advice is not intended as a treatment or therapy for disease.

  • Nutritional counseling, vitamin recommendations, and the adjunctive use of supplements are solely intended to upgrade the quality of the patient’s diet, to support physiological and biomechanical functions of the body.
     

We utilize whole food supplements and herbs to support healthy cell function and physiology. These supplements are recommended based on your unique nutritional evaluation. The cost of supplements is separate and not included in the evaluation fee.

3. Supplement Payment and Return Policy*

​

All supplements must be paid for in full at the time of purchase or before leaving the office.

 

Special orders—including supplements not typically stocked or currently out of stock—must be prepaid in advance.

To maintain the highest standards of quality and product integrity, Jones Chiropractic and Functional Medicine does not accept returns or issue refunds on any supplements. This policy ensures all products are stored and handled properly and guarantees the safety and efficacy of what we provide. We appreciate your understanding and cooperation.

4. Privacy and Confidentiality Policy

 

Safeguarding your privacy is paramount to maintaining the trust you place in us for your healthcare needs. Your patient records are treated with the utmost care, and access is restricted to authorized personnel only.

 

Written consent via mail or email is mandatory before any sharing of patient information occurs. If you have any questions or concerns regarding the privacy and security of your information, our team is available to address them promptly.

5. Appointment Policies*

 

Late Cancellation
Appointments cancelled with less than 24 hours' notice will be charged a $50 late cancellation fee.


No-Show
Missed appointments without any prior notice will incur a $50 no-show fee.


To ensure proper care and continuity:

  • Patients who have not been seen in over 12 months will require a Re-Examination Appointment – $150

  • Patients who have not been seen in over 36 months will require a Re-Activation Exam – $200


For patients out of state we offer virtual (online and phone) appointments to accommodate the distance. These evaluations ensure we can safely and effectively resume your care.
 

6. Financial Policy


You are responsible to pay for all services at the conclusion of your visit by cash, personal check, credit card, HSA, or FSA.

  • In the event of a bad/bounced check, a $35 fee will be charged to your account.


Insurance Disclaimer
Jones Chiropractic and Functional Medicine does not accept:

  • Health Insurance

  • Workman’s Compensation

  • Personal Injury Claims

  • Medicare or Medicaid (including supplemental policies)


If your insurance offers reimbursement for chiropractic care, you may submit your receipts independently using the information provided after your visit.

7. Additional Service Charges Policy*

 

Some cases may require additional therapies or services beyond your standard visit. In such instances, an extra charge may apply.

 

Our doctors do their best to inform you beforehand when a service incurs an additional fee; however, in some cases, this communication may be missed, and the extra charge will be added to your account and processed at the conclusion of your appointment.

In rare circumstances, a charge may be added after you have checked out. In these cases, the balance will be carried forward and must be paid at your next visit. We appreciate your understanding and continued trust as we work to provide the most comprehensive care possible.

8. Patient Conduct Policy​

 

Maintaining a respectful and courteous environment is essential for the well-being of both our team and fellow patients. We expect all patients to treat our team and each other with kindness and consideration. 

​

Disruptive or inappropriate behavior, which includes but is not limited to, verbal abuse, harassment, or any conduct that compromises the safety and comfort of others, may result in the termination of services. We believe that a positive and respectful atmosphere is fundamental to fostering a healing environment for everyone in our care.

9. Emergency Procedures

​

During office hours, if you experience a medical emergency, please seek immediate assistance from our team. They will provide the necessary support until professional help arrives. Your well-being is of utmost importance to us, and we are committed to ensuring a swift and effective response to any medical emergency you may encounter.

For after-hours emergencies, please contact 911.

​

10. Feedback and Concerns Procedures

​

We value your feedback and encourage you to share your experiences with us. Your insights help us continually improve our services.

​

Should you have any concerns or complaints, we request that you direct them to our office email for resolution at staff@drjoneschiropractic.com. We are dedicated to addressing any issues promptly and effectively, ensuring that your concerns are heard and resolved to your satisfaction.

11. Termination Policy

​

While we strive to provide care to all patients, Jones Chiropractic and Functional Medicine reserves the right to terminate services for those who consistently violate policies or engage in disruptive behavior. This measure is taken to maintain a safe and conducive environment for all patients.

12. Policy Change Notification

​

Jones Chiropractic and Functional Medicine reserves the right to update or change office policies, procedures, and pricing at any time without prior notice. However, we value transparency and will make every reasonable effort to provide at least 30 days’ notice of any significant changes via email, text message, or both.

We encourage patients to keep their contact information up to date to ensure they receive important communications in a timely manner.

13. Membership Terms and Conditions

 

Non-Covered Services and Additional Fees

All other services, goods, or appliances provided by Jones Chiropractic are not covered under this membership and will be billed separately if utilized. Pricing of other services can be recieved upon request.

 

Purpose of Plan

Diagnosis Codes Being Treated: M99.01, M99.02, M99.03, M99.04


Treatment Plan Summary: This membership covers the services listed below aimed at examining and treating for subluxations that might be sub-threshold of symptoms, maintain and continue patient's progress toward improved neuro-spinal health and increase overall health and well-being.

 

Payment Methods

Your Jones Chiropractic membership will remain active until it is cancelled. To maintain your membership, you must provide us with at least one valid and accepted Payment Method (debit, credit, HSA, or FSA card), which may be updated as needed. If a payment fails due to expiration, insufficient funds, or any other reason, and you do not cancel your membership, we may suspend your access to the memebership services until a valid Payment Method is successfully charged. By not cancelling your membership before the billing date of the next calendar month, you authorize us to charge your Payment Method for the membership fee for the upcoming billing cycle (refer to "Cancellation" below for details).

Additional Appointment Policy

If you require more appointments than those included in your membership during a calendar month, the cost for each additional appointment will be billed at the membership rate rather than the standard retail rate. This ensures that members continue to benefit from discounted pricing for any extra visits. Additional appointments will be charged at the time of service.

 

Membership Usage

Unused membership services do not transfer to the following calendar month. Each calendar month's visits must be used within that calendar month.

 

Membership Changes

Patients may switch between different membership options (if available) at the start of a new calendar month. A written request for switching memberships must be made at least 7 days prior to the start of the new month.

 

Membership Update Policy

Jones Chiropractic reserves the right to update membership terms, services, or pricing. Any changes to your membership will take effect no sooner than 30 days after providing written notice. Notification of updates will be sent to the contact information on file, such as email or postal mail. If you do not agree to the changes, you may cancel your membership before the updates take effect (refer to the "Cancellation Policy" for details). By continuing your membership after the update effective date, you agree to the revised terms.

 

Membership Pause Policy

You may request to pause your Jones Chiropractic membership for a period of up to 6 months without needing to cancel your membership. During the pause period, billing will be temporarily suspended, and you will not have access to membership services. To initiate a pause, a written request must be submitted at least 14 days prior to the desired pause date. Once the pause period ends, your membership and billing will automatically resume.

 

Cancellation Policy

This membership may be cancelled at any time with a written notice provided at least 14 days before the next billing cycle. No refunds will be issued for unused membership services within a month, as membership services do not transfer month to month. After cancellation, you will still have access to your membership services until the end of your current billing period.

 

Refund Policy

If the patient cancels the membership mid-month or before the next billing cycle, no refunds will be issued for unused membership services within a month, as membership services do not transfer month to month. You will have access to your membership services until the end of your current billing period. Any pre-paid monthly fees for future unused months will be refunded within 14 business days of the written request.

 

Request for Accounting of Funds

Patients may request an accounting of all funds used at any time. A detailed summary will be provided within 14 days of a written request.

 

Copy of Agreement

A signed copy of this recurring monthly membership will be provided to the patient upon execution of the agreement.

Retention of Records

A copy of this signed and dated membership will be maintained in the patient’s file as required by Chapter 4734-8-04 of the Administrative Code.

If you utilize all included visits in your package before the scheduled auto-renewal date, the package may be renewed early with your prior consent. Early renewal will begin a new start date on a new 12 month term under the same terms and conditions unless otherwise agreed upon in writing.
